Use this form only if you have already purchased a report or package. If you have purchased a COMPATIBILITY REPORT please fill out this form TWICE (once for each individual), remembering to include the name of the purchaser on both submissions.
In order to complete your order, please fill out the information on the following form as completely as possible. All data collected will be kept private and used solely for the purpose of calculating birth information and then communicating with you.
All reports are sent in PDF format and requires the free software Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader you can download it here for free.
If you know your TIME OF BIRTH, you can enter it in military time (00:00 to 23:59) or in the standard time (12:00 AM, 3:45 PM) with "AM" specified for morning and "PM" specified for evening. If you DO NOT know your TIME OF BIRTH, certain astrological aspects cannot be calculated. Don't worry, the majority of your chart can be calculated based on the date and location alone, but I'm not going to lie to you - it's better to have the time of birth, or even a general time of day. If you can locate this information, I HIGHLY RECOMMEND entering it below. If you absolutely cannot find your TIME OF BIRTH, I can still calculate your chart and reports without it.
